The purpose of this role is to promote and maintain the health and wellbeing of clients within the community setting and acute to community interface by utilising holistic and comprehensive assessment practices to formulate and implement care plans in consultation with the client and key internal and external stakeholders.

Pro rata remuneration package based on full-time equivalent value of up to $107,262 p.a. comprising salary range of $44.28 to $47.41 p.h. (Nurse Grade 6 – Band 1), recreational leave loading (17.5%), and employer contribution to superannuation (12.75%). Professional development assistance and salary sacrificing options may be available.

Appointment to this position requires proof of qualification and registration with the Australian Health Practitioner Regulation Agency (AHPRA) and possession of a current practising certificate. Certified copies of the required information must be provided to the appropriate supervisor/manager prior to commencement of clinical duties.
