This position based at Redland Hospital is a multidisciplinary role providing direct clinical care under the Transition Care Program. The Transition Care program is a jointly funded Commonwealth and Queensland Health Initiative offering older client’s time-limited and goal oriented Flexible Care Packages. The successful applicant will be required to supervise students, Personal Care Workers and Community Health Assistants.
The Health Practitioner or Clinical Nurse functions within the multidisciplinary Transition Care Team in providing direct clinical care and case management as part of an individualised care packaged provided under the Transition Care Program. The Health Practitioner provides support to other staff while promoting professional standards of practice and conduct, and contributes towards the achievement of unit goals and standards.

Occupational Therapist or Physiotherapist or Social Worker (Case Manager) or Clinical Nurse (Case Manager) is comprised of salary between $2345.40 – $3603.40 per fortnight (HP3 or Nurse Grade 6). Temporary full time position (up to January 2018).
