The interface of mental health service delivery is changing – and we are looking for Mental Health Professionals who want to be part of Medibank. In this role you will safely and efficiently manage a select case load of patients and facilitate the care for the individual between GP consultations to ensure all required activities are completed in adherence to the care plan as agreed between the patient and the GP. This may involve: mental health triage; risk assessment; care coordination; mental health education; referral to providers; crisis intervention; liaising with services and individuals involved in the care of callers; brief counselling, medication compliance and relapse prevention coaching and planning according to individual patient requirements.
This position will be based from our Perth office. Having an understanding of the health and social service network across the broader Perth metropolitan area will hold you in good stead in forming a strong resource for patients and GPs alike.
Key responsibilities include:
- Prioritise and respond to calls and be responsible for own caseload of patients;
- Focus on activities that targets recovery and relapse prevention
- Identify potential risk and proactively manage, escalating the response when required
- Delivery of regular updates to the patient’s primary care provider and maintenance of GP led care plan currency.
You will have demonstrated experience within the acute and/or community mental health sector and have knowledge of available support services. Your strong communication skills allow you to provide a triage and counselling experience using a number of approaches including solutions focused and strength based approach.
A relevant clinical qualification (3 years or greater) in the fields of Occupational Therapy, Psychology, Counselling, Nursing or Social Work is essential as is solid experience dealing with acute mental health issues and disorders.
