An opportunity exists within the Aged Care Assessment Service for and enthusiastic and passionate clinician with sound clinical knowledge and experience in aged care. You will conduct community assessments and have the ability to work autonomously as well as in a team environment.

Eastern Health operates from many different locations, our largest sites include Angliss Hospital, Box Hill Hospital, Healesville and District Hospital, Maroondah Hospital, Peter James Centre, Turning Point Alcohol and Drug Centre, Wantirna Health, Yarra Ranges Health, Yarra Valley Community Health. Although this position is primarily based at the Peter James Centre there may be the requirement to travel to other Eastern Health sites.

The Central East ACAS is one of the largest in Victoria, providing comprehensive aged care assessments to clients living in the municipalities of Monash, Whitehorse and Manningham.

An understanding of the roles and responsibilities of an Aged Care Assessment Service is an advantage.

The ACAS program offers a comprehensive and supportive orientation and training program for all staff.

To be successful in the role you will have:

  • Recent experience in Aged Care
  • Experience in a community setting
  • Well developed assessment and care planning skills
  • Ability to work as part of a diverse team
  • Strong commitment to high quality patient care
