  • About the facility

    Barwon Health has five major community health centres, Corio, Belmont, Newcomb, Torquay and Anglesea which provide a range of services to the Barwon region. Services provided by the Newcomb Community Health Centre include podiatry, physiotherapy, psychology, social work, nutrition, speech pathology, community health nursing and occupational therapy. Barwon Health Community Nursing is a HACC funded service and also provides care for clients requiring Hospital in the Home, Post-Acute and Palliative care.

    Working within a multi-disciplinary team environment, this position focuses on the provision of high quality, carer support services that are responsive to the needs of the local community. This position encompasses a variety of roles including information provision, guided referral and the delivery of high quality, innovative carer support services.

    Selection Criteria
    Essential: Appropriate qualifications in relevant health related field: Social Work, Welfare, Aged Care, Mental Health, Disability & Community Case Management
    Essential: Current Victorian Driver’s Licence.
    Essential: Current Police check/ updated 3 yearly
    Essential: Highly developed interpersonal and communication skills (written and verbal).
    Essential: Highly developed Skills and knowledge in the use of Content Management Systems, Microsoft Office applications and database systems.
    Essential: Demonstrated commitment to further education and learning.
